y - y1 = m(x - x1)

y - 6 = (1/2)(x - (-3))

y - 6 = (1/2)(x + 3)

what do you need? I assume you need to write an equation if so then
y - 6 = 1/2(x +3)
y - 6 = 1/2(x) + 3/2
y = 1/2(x) + 15/2

equation in sloep intercept form:
y = 1/2(x) + 15/2
